You will need to provide:

  • Letter from a licensed psychologist recommending surgery;
  • Documentation (prescription, letter) indicating that you have been taking hormones for at least 1 year.
  • Lab Tests: chest X-ray, EKG, CBC, blood sugar, HIV, HBsAg, UA, Bun/Cr, electrolytes, LFT.
